Case Management Programs
Brown County offers multiple Case Management Programs. All are based on program specific eligibility and assessed by a member of our team.  The person seeking services needs to be involved in the referral.

Comprehensive Community Services (CCS) 
CCS is a Medicaid-funded program to support mental health and/or substance use needs.  Eligibility is based on a functional screen, and a need for services beyond outpatient treatment.  CCS provides service facilitation (case management), psychotherapy and substance use counseling, and community-based services to work on recovery goals. Services are provided by teams of professionals, all coordinated by a CCS service facilitator.

Community Support Program (CSP)  
CSP is an intensive case management program for individuals with a persistent and severe mental illness diagnosis. CSP is provided by an interdisciplinary team of professionals, such as psychiatrist, nurse practitioner, psychotherapist, case manager, and registered nurse. Services provided may include crisis intervention, psychotherapy, medication management, psychiatric/psychological services, employment-related services, social/recreational skill training, and activities of daily living services. 

Targeted Case Management (TCM) 
TCM assesses the impact of mental health and substance use concerns on daily functioning, refers to community resources and providers, and monitors effectiveness of services.   Case managers also monitor civil commitments.  

Homeless Outreach/Intervention    
Case managers can assist individuals experiencing homelessness streamline access to mental health and substance use services, as well as other resources in the community. The case managers utilize resources for housing that can be found at Brown County Homeless & Housing Coalition.
