Comprehensive Community Services (CCS) is a Medicaid funded program for people of all ages to live their best lives. CCS provides service facilitation (case management), psychotherapy and substance use counseling, and community-based services to work on goals related to mental health and substance abuse. Eligibility
The child must:

• Be eligible for Wisconsin Medicaid.
• Live at home, foster care, or other similar setting.
• Be willing to participate in voluntary services.
• Meet with a coordinator to complete the state screening tool to determine eligibility.
• A need for mental health and/or substance use services beyond outpatient treatment. 

Your team
Individuals who are eligible for the CCS program work with a Service Faciliator, who is an employee of Brown County Health & Human Services. They help children and families find the CCS services they need. Individuals interested in therapy will need to see a therapist affiliated with CCS. A list of CCS therapists can be reviewed during initial contact with our program. 
The Service Facilitator will meet with the child and family to complete a comprehensive assessment of strengths and needs. Based on that assessment, a service plan will be created with goals and objectives. Contracted service providers may be utilized to provide support services. The team will meet at least every 6 months to review the plan and goals.

Note: If the individual is currently involved in a Day Treatment program or residential program, please contact us when a discharge date is determined.
