Children's Long Term Support
The Children's Long-Term Support (CLTS) Program is a voluntary program that assists children with disabilities and their families through supports and services. CLTS funds home and community-based services for kids with disabilities, with the goal for children to grow and live their best lives. Also known as “Waiver”, this program allows states to use Medicaid to fund additional non-medical supports and services not normally offered through insurance.
Eligibility:
The child must:
- Be eligible for Wisconsin Medicaid.
- Live at home, foster care, or other similar setting.
- Meet with a coordinator to complete the state screening tool to determine eligibility.
- Be able to get safe, required care at home or in the community.
Cost:

Your team:
Kids who are eligible for the CLTS program work with a Support and Service Coordinator, who is an employee of Brown County Health & Human Services. They help children and families find the CLTS services they need.
Your Support and Service Coordinator will meet with your family to help identify outcomes and develop a service plan specific for your child and family’s needs.
