Children’s Community Options Program (CCOP)
The Children's Community Options Program (CCOP) helps children with special needs and their families achieve their goals. The program provides supports and services that help children become more independent and participate more fully in their communities. CCOP pays for many supports and services that you can’t get through other insurance, Medicaid, or the Children’s Long-Term Support Program (CLTS).
 
The Program Offers
Information and help in finding services and maximizing community resources for children with disabilities
Limited funding to buy needed services and goods that can’t be bought through other sources including the CLTS program
Helps in linking families with other families to strengthen natural support
Can pay for one-time needs 
How Dose the Program Work
Families apply for services through Brown County Human Services. The referral process includes eligibility determination thought a functional screen. Information for the functional screen is gathered from a variety of sources which include interviews with the family, educational records, and medical history.
 
Once eligibility has been determined, a Support and Service Coordinator (SSC) schedule a home visit. Together, the SSC and the family will develop an Individual Service Plan (ISP) that addresses the child’s unique needs. Identifying what supports and services can be funded through the CLTS program and what service can be funded through the CCOP program.
 
CCOP is not an income basis program, however a parental payment may be required depending on family size, income and cost of services identified in the Individual Service Plan (ISP). Because each County has limited funding for the program, eligibility does not guarantee a family will receive services. Agencies may have waiting lists for services and assessments. 

Who is Eligible
Children and their families who meet all the following requirements are eligible to receive goods and services on a first-come, first-served basis within the limits of CCOP funding:
The child has a disability and meets an eligible level of care as determined by a functional screen
The child is under age 18 or under 22 and does not qualify for adult long-term care*CCOP is a statewide program, so children are eligible for services in their county of residence.
