Service Provider Contracting
Thank you for your interest in becoming a provider of services for clients in our community.  Brown County Health and Human Services (HHS) serves a variety of client populations through a diverse array of units and programs including, but not limited to:
 

Child Protective Services (CPS) – Promoting the safety and well-being of children/youth and their families.  Service needs may include parent training, counseling/therapy, respite and child care.

Youth Justice – Holding youth accountable for violations and helping them develop skills and competencies to live responsibly and productively.  Service needs may include counseling/ therapy, AODA services, daily living skills training and mentoring.

Children’s Long-term Support (CLTS) – Providing support and services to children/youth with disabilities and their families so they can remain in their home and community.  Service needs may include respite, daily living skills training, health and wellness, parent training, and transportation.  CLTS has separate requirements dictated by DHS, and therefore, a separate provider agency registration document.

Comprehensive Community Services (CCS) – Providing psycho-social rehabilitation services to both adults and youth experiencing mental health and/or substance abuse issues.  Service needs may include skill development, wellness management, employment training, medication management and psychoeducation. 

Coordinated Service Team (CST) –  Providing individualized supports and services in a team approach for children/youth experiencing complex emotional, physical, and social challenges.  Service needs may include parent training, counseling/therapy, daily living skills training and mentoring.

Child & Adolescent Behavioral Health (CABHU) – Providing psychotherapy and support to children and youth with complex and severe mental health and emotional challenges.  Service needs may include specialized inpatient services, daily living skills training and mentoring.

Adult Behavioral Health – Assisting adults with chronic or persistent mental health needs to gain the skills and competencies to live as independently as possible.  Service needs may include counseling/therapy, AODA services, daily living skills training, employment training and in some cases, residential care.

Adult Substance Abuse Services – Providing support to adults with substance abuse needs through treatment and recovery.  Service needs may include residential AODA treatment, residential sober living and counseling/therapy.

Adult Protective Services – Protecting the safety of vulnerable adults experiencing abuse, neglect or exploitation.  Service needs may include guardianship, supportive home care, and in rare cases, residential care.

Adult Treatment Courts – Supporting adults in the criminal justice system with long-term recovery needs through a specialized model wherein judiciary, prosecution, defense, probation, law enforcement and treatment communities all work together.  Service needs may include residential sober living and counseling/therapy.

  2. Review any service specification documents relevant to the services you wish to provide (links below).
  3. Complete the Purchase-of-Service (POS) Provider Agency Application (link below), save and e-mail to the address at the bottom of the document.
  4. Wait to be contacted for follow-up and next steps (this may take up to a few weeks).
